ROLE AND RESPONSIBILITIES

Care of clients in all areas of treatment, to ensure their comfort and well being. Including, but not limited
to, daily assessment of their physical and mental state, upkeep and accuracy of their charts.

BRIEF OUTLINE OF POSITION:
1. Responsible for taking vital signs, assessment of client’s medical and psychological symptoms
and the documentation of same in the nursing progress notes.
2. Ensure proper and accurate transcription of physician orders and that accurate documentation is
maintained on the administration of medication, verbal physician orders and individuals that are
on self-medication plans.
3. Inform the physician of acute or ongoing medical and/or psychiatric problems scheduled clinic
appointment times and the results of medical assessments and testing.
4. Schedule outside medical appointments and ensure client’s transportation.
5. Review client’s health plan, develop and manage medical and medication goals for clients’
comprehensive treatment plans including clients who are in the self-administration medication
program.
6. Develop and run nursing groups, act as co-leader in group therapy and participate in treatment
team meetings.
7. Act as liaison between the client’s physicians and the staff.
8. Provide training to clients and staff in the areas of hygiene, medication side effects, first aid,
nutrition, etc.
9. Other duties as assigned by Associate Director.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
Prefer current Pennsylvania R.N. license, experience working with chronically mentally ill adults,
preferably in a residential setting or Pennsylvania LPN with extensive clinical experience.

WORK AUTHORIZATION/SECURITY CLEARANCE (IF APPLICABLE)
Must have an acceptable PA criminal history, FBI clearance and Child Abuse clearance done prior to hire.

WORK ENVIRONMENT
This job operates out of the LTSR location.

POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position, Monday to Friday 4pm to 12am.

PHYSICAL DEMANDS
Must have physical ability to: A) Assist with lifting of clients; B) Restrain clients in emergency situations;
C) Kneel and bend
