Job Description

Position:Direct Service Professional (DSP)

Department: Intellectual Disabilities

Reports to:CPS Program Specialist/Director

Hours: Monday to Friday 8:30am to 4:30pm

Brief outline of position:

1.Lift, carry, or transfer program participants with assistance (2 person transfers)

2.Assist participants during lunch (this includes feeding for those who require the assistance to eat)

3.Assist participants in the restroom according to their individual needs

4.Assist participants to explore opportunities in the community. The goal is to increase socialization, community involvement, and community safety awareness

5.Enter progress notes daily on each participant you worked directly with

6.Provide physical assistance as needed during personal care, program activities, and during community activities

7.Implement activities identified in the ISP and by Program Director

8.Accompany participants on accessible public transportation

9.Complete 24 hours of training annually including CPR, First Aid, Everyday Lives, Individual Service Plans, and all trainings required by the 6100 regulations

10.Follow all behavior plans as written

11.Develop and implement program activities based on participants interests and abilities

12.Assist and support all training of new staff members

13.Role model appropriate social skills

14.Develop professional rapport with all participants in program

15.Perform other duties as assigned by the Program manager

16. Be able to work Monday to Friday 8:30am to 4:30pm

Minimum requirements:

High School Diploma and experience in a healthcare field. Must be physically able to climb stairs, take public transportation, and lift 100 pounds (2 person transfers)
