Care Coordinator

Always Best Care is actively seeking a Care Coordinator to
join our team. The ideal candidate for this position is an experience,
organized and dependable. The right candidate for this job is responsible for
the coordination of services between clients and caregivers.

Duties &

Provide on-site coaching, support, and education
of Direct Care Workers.

Enhance the quality of care of all Always Best
Care clients.

Deliver direct care worker training in the areas
such as, but not limited to medication assistance and the safe use of
mechanical lifts.

Complete in-home care plans on all current &
new clients, as well as follow-up on client complaints.

Function in the capacity of a Home Health Aide
when needed


Willing to travel within city of Philadelphia
& surrounding counties

Ability to work independently in the field

Capability to make effective and persuasive
speeches and presentations to key audiences

Skilled at working with different personality
types in a professional manner

Commitment to confidentiality of protected
client health information

Good interview and interpersonal communication

Proficient in E-mail, Internet use, Microsoft
Word, Excel and Power Point

Attention to detail

Strong organizational skills

Ability to manage & meet deadlines

Ability to think on his/her feet and adapt
quickly to respond key referral sources questions and various situations

Punctual attendance to work/office or client’s

Maintain current CPR

Position Type/Expected
Hours of Work

This is a full time hourly position with a Monday through
Friday, 8:30 am – 5:00 pm and every other weekend work schedule.


This position requires up to 50% travel, and will require a
valid driver’s license, reliable transportation and insurance coverage that
meets state guidelines.

Required Education
and Experience

High School Diploma or GED equivalent required

At least two (2) years of hands-on care giving
experience, working with seniors preferred

Bilingual a plus
